Participation in the Dental Saving Plan (the “Plan”) is only available to Patients without Dental Insurance. Membership is restricted, is not transferable to another patient, or applicable in any other dental office. Annual Membership Fee must be paid in full to receive Plan benefits. Applicable fees for additional services for Plan participants must be paid at the time of services. Plan related discounts can not be combined with other offers/discounts. Patient’s responsibility to schedule and keep appointments. The Annual Membership Fee is non-refundable under any circumstance. Plan participants’ rights are forfeitable. Plan Participant’s contractual agreement is with Debra C. Duffy, DDS, PA. All statements in the forgoing explanation are qualified by the terms and conditions of the Plan itself.
